Under general or regional anesthesia, the femoral nerve and its branches are microscopically exposed through an accessible approach. Common practices include layered dissection, identification of fibrous bands or scar compression points, microscopic release of the peripheral nerve, and, if necessary, neurolysis or interposition of spacers to reduce friction. Intraoperative neurophysiological monitoring can be used to identify functional fibers. After hemostasis and irrigation, the layers are sutured and pressure dressing is applied. Postoperative monitoring of pain, sensation, and muscle strength is performed.
