SCS uses adjustable electrical stimulation to modulate the pain pathways in the dorsal column of the spinal cord. A common approach is to first perform a trial stimulation, percutaneously placing electrodes into the epidural space and connecting them to an external stimulator to assess pain coverage and tolerance. If suitable, a subcutaneous pulse generator is then implanted and connected to fixed leads, with intraoperative imaging and electrophysiological localization. Postoperative monitoring of the wound and stimulation parameters is performed, gradually optimizing to reduce pain interference and improve function.
